What happens to the cost of producing a competent therapist?
This is the uncomfortable question mental-health startups and platforms must confront
The mental-health industry faces a severe affordability and access crisis, driven by massive, treatment gaps and low public funding. But there are as of today, 510 registered start-ups in India solving this problem of affordability through digital health apps, aggregation, counsellors on low-cost and AI agents acting as 24/7 triage tools.
Sounds encouraging, doesn’t it? But is affordable access equal to relevant care by good clinicians? We keep asking how to make therapy cheaper. Perhaps we should first ask who is paying the price. Increasingly, I wonder whether we are solving it by quietly transferring the cost from the system to the therapist.
A therapy session may be priced as one hour of professional time. But that hour is rarely one hour of work. Behind it sits assessment, formulation, clinical documentation, treatment planning, supervision, reading, continuing education, administration, cancellations, case consultation and an enormous amount of emotional labour that rarely appears on the invoice.
Yet when we talk about making therapy “affordable”, most conversations begin and end with the client’s fee.
Therapists who undercharge often compensate by seeing more clients. More clients mean less time for supervision, reflection and advanced training. Eventually, professional development becomes a luxury rather than a clinical necessity.
And then we wonder why mental healthcare struggles to produce a consistent, highly trained clinical workforce.
This is not an argument that therapists should simply charge more.
That would solve one problem by creating another: therapy becoming inaccessible to people who cannot afford higher fees. Clients need affordability. Therapists need sustainability. These are not opposing goals. They are two sides of the same healthcare problem.
We have perhaps made a conceptual error by treating the therapy session as the entire product.
It isn’t. The product is the clinical capability behind the session.
A competent therapist is built through education, supervised practice, reflective work, exposure to complexity, continuing professional development and years of clinical experience. None of that happens for free. Healthcare became scalable not by asking doctors to work endlessly for less. It built infrastructure around them-hospitals, laboratories, medical education, specialist training, insurance and technology.
Psychotherapy needs to think similarly.
If mental-health companies want therapy to scale, they cannot build only marketplaces that generate more appointments for therapists. Think of this, if we make therapy cheaper by making therapists poorer, have we made mental healthcare more accessible or have simply transferred the cost to a struggling young therapist.
If the clinician delivering it is exhausted, under-supported and unable to invest in becoming better, the system may have reduced the price without improving the care.
We should stop asking only, “How do we make therapy cheaper?”
We should also ask: “What would it take to make excellent therapy sustainably affordable?”
That is not merely a pricing question.
It is an infrastructure question. And perhaps it is the question the next generation of mental healthcare needs to answer.
What’s the solution? Let’s get ambitious and redesign mental healthcare leadership in India. Here are some thoughts – Maybe we create structured university pipelines, smooth transition from psychology degrees into clinical training. How about sponsored seats – hospitals, NGOs, corporates and foundations fund therapist training. Or continuing education financing- low-interest loans / deferred-payment models specifically for clinical training. Let’s think paid clinical residencies, therapists earn while they train rather than choosing between income and development. And may be even employer-funded CPD – hospitals/platforms allocate annual professional-development budgets. We can work on supervision subsidies -pooled institutional funding makes quality supervision affordable. Add to the mix, hospital–academy partnerships – shared training, supervision and clinical placements. Tiered career pathways that follow clear progression from trainee to supervised therapist to advanced clinician to supervisor. With public–private scholarships and government plus philanthropy funding for underserved regions and populations. Or let’s borrow from the European model of research-to-practice funding -fellowships that connect clinical practice with Indian mental-health research.
Now imagine a Guild of psychotherapists built not merely to train therapists, but to solve the national crisis of creating and sustaining therapeutic capability.
