Introduction: Why Health Insurance Claims Get Rejected

There are many reasons why people have concerns about getting a health insurance claim rejection. Many people have had this type of experience with their health insurance provider after paying premiums regularly and shopping around for the best health insurance plan for themselves, only to have claims denied in a medical emergency.

Claim denials often create financial strain and can delay medical treatment. Policyholders will be able to ensure that the process goes more smoothly for them through any health insurance, and also through any of the best health insurance policies available to them.

Nearly every health insurance policy comes with waiting periods that apply to maternity benefits, some diseases or injuries, and any existing medical conditions before they can receive any benefits under any of these categories. Knowing exactly what is included in your health insurance policy helps you make the most informed decisions in the event of an emergency.

2. Be Honest about All Medical Information

All aspects of a person’s health history (including past medical situations), their lifestyle (including any potential health risk behaviours), and current medical treatments should be disclosed when applying for your health insurance policy. When a person gives all the required details about themselves, they increase the trustworthiness of their health insurance policy if an unexpected medical event occurs.

3. Use In-Network Hospitals

Health insurance providers typically have large networks of hospitals that help streamline the process for issuing cashless claims and minimise mistakes (the primary reason for denial of claims). The majority of the best health insurance providers offer a large range of network hospitals throughout large urban areas.

4. Gather Medical Information

Collect all your medical records (including reports) as well as everything that has been prescribed or billed, along with the discharge summary you received upon leaving the hospital. Proper documentation helps ensure that there are no delays in processing your claim, so that you won’t lose your claim through the health insurance company.

A health insurance claim rejection is not the end of the road. Many claims get approved upon review or appeal.

1. Understand Why the Claim Was Denied

When a claim is denied, the insurer provides written notice to the insured as to why the claim was denied. Carefully review the reason for your denial, and determine if it is due to a failure to provide documentation, ineligibility, waiting period, or an exclusion under the terms of the policy.

2. Resubmit All Missing Documents

If the reason for your denial was a lack of information, then your denied claim should be granted by providing the requested additional documentation; if the lack of documentation causeda health insurance claim rejection, then submitting the missing documentation will change the outcome of the claim.

3. File an Appeal

Insureds have the right to file an appeal with the insurer’s grievance department if their claim is denied; even those insured with some of the best health insurance may have had problems with claims that were resolved at the appeal stage.
