A High-Risk Pregnancy with Severe Hypertension and Intrauterine Growth Restriction Successfully Managed

Pregnancy is usually a period of joy and anticipation; however, certain medical conditions can turn it into a high-risk situation requiring expert supervision and timely intervention. One such serious condition is severe hypertension during pregnancy, which can adversely affect both maternal and fetal health. When associated with severe intrauterine growth restriction (IUGR), the risk to the unborn baby increases significantly.

At Sri Sri Holistic Hospitals, Kondapur, a critically high-risk pregnancy complicated by severe hypertension and fetal growth restriction was successfully managed under the expert care of Dr. Komal Mettupalli, Obstetrician & Gynaecologist, through prompt diagnosis, continuous monitoring, and timely surgical intervention.

Clinical Background

The expectant mother was admitted with severe pregnancy-induced hypertension, a condition known to reduce placental blood flow and increase the risk of serious maternal and fetal complications. Subsequent clinical evaluation and imaging revealed that the fetus was affected by severe intrauterine growth restriction, indicating compromised fetal growth inside the uterus.

Recognizing the seriousness of the condition, the pregnancy was classified as high-risk and placed under strict antenatal surveillance under the guidance of Dr. Komal Mettupalli, Obstetrician & Gynaecologist, Kondapur.

Decision for Emergency Intervention

Despite optimal medical management, the mother’s blood pressure remained critically elevated, and fetal growth parameters showed concerning trends. In view of the potential risk to both mother and baby, the treating team, led by Dr. Komal Mettupalli, decided to proceed with an emergency Lower Segment Caesarean Section (LSCS).

The decision was made in a timely manner to prevent further complications and ensure the best possible outcome.

Delivery and Neonatal Outcome

The emergency LSCS was successfully performed, and a preterm baby weighing 900 grams was delivered. The newborn was immediately shifted to advanced neonatal intensive care for specialized support and continuous monitoring.

Despite the extremely low birth weight and high-risk background, the baby responded positively to neonatal care.

Post-Delivery Progress

With comprehensive postoperative care and multidisciplinary support, the mother’s condition stabilized well following surgery. The newborn continued to show gradual improvement under expert neonatal supervision.

At present, both the mother and the baby are stable and doing well, reflecting the effectiveness of coordinated obstetric and neonatal care.

Significance of Timely and Specialized Care

Severe hypertension associated with intrauterine growth restriction remains one of the leading causes of maternal and neonatal morbidity if not managed appropriately. This case highlights the importance of:

  • Early identification of high-risk pregnancies
  • Close maternal and fetal monitoring
  • Prompt surgical intervention when indicated
  • Availability of advanced neonatal care

According to Dr. Komal Mettupalli, early diagnosis and timely decision-making are crucial in achieving favorable outcomes in such high-risk pregnancies.

Conclusion

This successful outcome reinforces the importance of expert obstetric management and advanced neonatal facilities in handling complex pregnancy cases. With vigilant monitoring, timely intervention, and dedicated postnatal care, even the most challenging high-risk pregnancies can be managed safely.
